RMA Request Form

Return Merchandise Authorization (RMA #)

Follow these steps to request an RMA #:

1. Step 1: Complete the RMA Request Form in full
   *RMA numbers will not be issued without a completed form.
   • Click to Download RMA Request Form
   • Email your completed form to request service
   • After initial inspection a formal quotation will be emailed to you, at that time we will request a purchase order or credit card for payment of your repair.
   • There is an inspection fee of $150.00 for RMAs.
2. Step 2: Pack Your Shipment
   • DO NOT USE STYROFOAM PEANUTS
   • Do not ship instruments in wooden crates:
     Additional fees will be applied to shipments received in wooden crates. 
   • If you do not have original Thwing-Albert packaging for your instrument, quotes for replacements are available.  Email request to request service. 
3. Step 3: Clearly mark the outside of your box with your assigned RMA #
4. Step 4: Ship RMA Items, Freight Prepaid to:

   *Include a copy of your completed RMA Request Form in the package.
